Here are the applications for Credit Reports for State and County Licensing requests.

Payment is by credit card ONLY.  The charge is $40.00 per report PLUS a credit card transaction fee which is usually $2.00.

Reports are usually completed the same day or the next day.  One copy is mailed to you and another copy is mailed directly to the State or County licensing board.

If you want a copy faxed to you, there is an additional $1.05 faxing charge and you MUST state IN ADVANCE that you want it faxed.  If you do not want it faxed, the charge for the report is $40.00 plus a $2.00 credit card transaction fee.
