Here are the applications for Credit Reports for State and County Licensing requests.

Payment is by credit card ONLY.  The charge for an individual report is $35.00 per report PLUS a credit card transaction fee which is usually $2.00.  The charge for a business report is $41.00 in Florida and $52.00 out of Florida plus a credit card transaction fee of $2.05 or $2.55 out of state.

Reports are usually completed the same day or the next day.  One copy is mailed (US Post office) or emailed or faxed to you and another copy is mailed directly to the State or County licensing board.

Reports are in compliance with Local, State and Federal Public Records check as required by the DBPR.
